The Illinois Department of Healthcare and Family Services announced Friday the personal information of more than 3,000 of its clients may have been compromised because of a contractor's computer error.
According to a release issued by HFS, identification cards intended for Family Health Network contractors were mailed to an incorrect address as a result of a computer program error. HFS was notified of the mistake on May 10.
As a result, 3,133 HFS clients may have had their names, state-issued Medicaid numbers and birth dates disclosed, according to the release. Social security numbers and home addresses were not listed.
HFS is in the process of notifying all clients that may have been affected by the incident, according to the release. The department is encouraging anybody who is concerned about identity theft to register for free fraud alerts with consumer protection agencies.
